Zoom on the screen who is the fairest of all…. The surge in Plastic surgery during Covid.

The pandemic has made a drastic change in our life, due to frequent usage of social media and apps for both work and leisure activities. This lockdown and quarantine life has made people stare at mobile phones and PC screens for consecutive hours for video calls, virtual meetings, and get together.  People learned to stay home but stay with their devices more than ever, from working at home, attending meetings, virtual classes, streaming videos, playing games to online shopping. Research by the Eyesafe company showed that during the pandemic, adults have spent more than 13 hours per day on average which was 9 hours in 2019.

What is the effect of over-time exposure to screens?

Smartphones, laptops, and such devices transmit blue light, which is toxic for the eye retina, causing vision impairment. Continuous exposure to blue light merely kills the retina cells, leading to macular degeneration. Dry, itchy, or even painful eyes; blurred vision, burning, stinging, redness; watery eyes, a foreign body on the eye surface are the basic symptoms of Digital-related eye strain.

Has Pandemic video calls made us scrutinize ourselves more?

Ms Radhika (name changed) a senior manager in the reputed company came to me for consultation, Her concern was ” I see a lot of wrinkles and lines around my forehead and eyes which bothers me a lot in video calls than in real life”.

The long-duration video calls make us stare at the video thumbnail, noticing the flaws in our face we never saw before. It is because you never see your face while talking directly to someone. So certain people seem to scrutinize themselves a lot during zoom calls and start analysing their facial features. Things like crows feet, wrinkles along the forehead and eyes, crooked nose, lip shape or size disparity, or balding of the hairline, the contour of the jawline, folds along the neck has become more noticeable to oneself now.

Psychologists have reported that seeing oneself over and over again during the video calls leads to obsession over the body image. Perceptual distortion occurs when the flaws in our body are highlighted and disproportionately focussed on until it becomes magnified in our perception. Zoom boom is just a continuation of the trend that started with a selfie. It was called selfie dysmorphia, now it is less to do with photos and more to do with video calls. What initially was thought of like a celebrity or a beauty industry demand is slowly catching up with regular people due to video calls and self analyses.
Plastic surgeons and cosmetic doctors around the world have reported an unprecedented surge in non-surgical and surgical procedures during the lockdown. The other reasons for the surge in cosmetic procedures worldwide include pent-up demand after months of lockdown, WFH  being an opportune moment to keep the surgery private, having good recovery downtime at home,  usage of masks to cover any postoperative swelling and no social meetings during recovery are the other reasons for the surge in plastic surgery during Covid times.

Most sought after treatments include fillers, botox and skin resurfacing which reduces the lines that we notice during a video call. Other procedures for which there is a surge include eyelid rejuvenation, blepharoplasty and rhinoplasty, facelifts, neck rejuvenation and neck lifts.

  • Botox:  For crows feet, frown lines around the eyes
  • Fillers: Hyaluronic acid Fillers -For tear troughs or depressions under the lower eyelid causing a tired look to the face.
  • Blepharoplasty:

Saggy eyes, puffiness of lower eyelid, festooning of eyelids, the droopy brow is a natural ageing symptom and you may prefer surgery if you are really conscious and bothered about your appearance. Certain lifestyle trends like improper food habits, lack of sleep, smoking also lead to droopy eyes, sagging eyebrow and dull looks. Blepharoplasty at times goes beyond cosmetic concerns. You can also choose the surgery if you are suffering from vision impairment due to saggy eye muscles or eyelids.

Blepharoplasty is basically performed on an outpatient basis where people can recover fast after the procedure. The surgeon would inject a numbing agent into the eyelids before the surgery for anaesthesia.  Excess skin is removed, fat is repositioned,  and orbital tissues are tightened. Eyelid swelling, bruising, dry eye and discomfort after the surgery is managed with medications.

If you are concerned about your facial flaws or you have vision-impairment due to your sagging eye muscles or skin, consult an expert. Before you go for treatment take advice on if you are really fit for the same with all physical conditions. Make sure you opt for safe and secure treatment without any after-effects. Make sure all the covid protocols are taken care of during the procedure.

Zoom-inspired plastic surgery may be a new reality that we need to be aware of, however, I would caution the judgement it elicits on our looks and not over scrutinize ourselves. The mirror on the wall is not always flattering.

When it comes to cosmetic surgery, breast augmentation is one of the techniques that probably lead the long list. Since from the early 1960’s, women were flocking to plastic surgeons to get this procedure done. What Should I Expect During a Consultation with the Surgeon?

Amid your breast augmentation consultation, be prepared to speak about:
  • Why you need a breast augmentation surgical procedure, your expectations and the desired results
  • Current medications, tobacco, alcohol, and drug use

During the Phase of Recovery…
During the recovery phase of your breast augmentation surgery, your bosoms will be wrapped in gauze dressings and a support bra or an elastic bandage will restrict the possibility of swelling and support the breasts well, as they heal. You will be given specific postoperative directions for your breast implant recovery and a follow up appointment with your surgeon will be scheduled. You may also get medications or prescription, which may include pain killers and antibiotics to prevent any possible infections. You might be allowed to go home when stable for discharge, normally at the end of the day.

What Results can be Visible After the Surgery?

Rhinoplasty – What to Expect..?

The promise of Rhinoplasty surgery is found to entrance a great number of individuals today, to seek out procedures to enhance their appearance, fix a birth imperfection, or resolve breathing issues. As with every other surgery, it is remarkably essential for rhinoplasty to ensure that both the patient and surgeon are on the same page as far as the situations that should be avoided after surgery, expectations on the recovery time and physical constraints are considered. It’s highly imperative to schedule your surgery in such a way to allow enough recuperation time, so that you can return to your normal activities accordingly. So, in this article, we will talk a tad about those expectations, you should have before having a rhinoplasty.

Pre-Surgical Preparations
Prior to your surgery, it is important that you start preparing your body to encourage the healing process. Ensure that you refrain from any smoking habits (in case if you have), as smokers have the chance of slow healing and can even lose their skin amid the healing process. Also, stop alcoholic consumption. Don't take aspirin or aspirin compounds, or any medications like vitamin, hormone, or birth control pills, until a week after the surgery, to improve your healing time by reducing any possible swelling and bruising.

The Day Before Rhinoplasty
Try not to eat or drink anything after the midnight before your rhinoplasty procedure; this will halt regurgitation when you are under anesthesia. Now, on the day of the surgery, you can take your everyday medicines (kindly have a talk with your physician) with just a sip of water.

Post-Surgical Recovery
Rhinoplasty requires a couple of months to completely heal; so, make sure to plan your social engagements in like manner. There are chances for you to encounter post-operative depression, during the initial one to two weeks after the surgery; this is normal and should pass quickly as its own. Seek the help of your physician in the event that your anxiety or depression does not decrease over time. Also, be aware that the ultimate outcomes may not be visible for six months to two years.

Swelling, Bruising and Numbness
Swelling and bruising of the eyelids, nose, and cheeks are normal during the first two weeks after the surgery, and it is extremely common for there to be bulbous swelling at the tip of the nose, which is the most heavily-operated area. No need to fear about this – the swelling will get reduced over time. What you can do from your side to reduce the inflammation of your eyelids is to use iced gauze pads every hour in the initial two days post-surgery (however, be cautious to keep your nose dressings dry).
Likewise, be informed that the swelling inside your nose can cause nausea and breathing issues for the first week. So, during the initial recovery time, expect to inhale and sneeze from your mouth. It is recommended that you sleep on two or three pillows to reduce the possibility of swelling and for easier breathing.
A few patients are also found to experience difficulty to smile because of the numbness and swelling during the initial couple of weeks after the surgery. No worries, your smile will come back to ordinary after the swelling subsides.

Aftercare
It is imperative for you to follow your physician’s specific aftercare guidelines, for a speedy recovery. Make sure that you give enough care for the incisions that are made inside your nose during the procedure. Moderate bleeding is normal for the initial few days, so dress your nose with gauze, and change as frequently as required. Other things to be taken into consideration are discussed below:

·       Diet: Eat a light and cool diet as endured for the first 48-72 hours after the surgery; drink a lot of fluids. Try not to consume anything spicy or hot in temperature, as such foods will dilate your blood vessels thereby resulting in swelling.
·       Use Sunscreen: Keep your nose completely out of the sun for 4-6 weeks. Try to use a sunscreen for the first six months after surgery whenever you go outside.
·       Exercise Limits: Avoid strenuous activities like jogging, or weight lifting for the first four weeks of recovery, as this may result in increased heart rate, thereby increasing your blood pressure, finally making blood clots that have formed to help the healing process to ‘pop off’ resulting in bleeding and swelling.
·       No Smoking: Absolutely do not smoke for four weeks after your surgery. The nicotine in the tobacco is a vasoconstrictor that cuts off the blood supply to your tissues, in this manner, leading to scarring, infection and increased healing time.

So, now, it’s time to commit to Rhinoplasty! Make a consultation appointment with Dr. Sandhya Balasubramnyan, and we will discuss the best option for you!

Breast Reduction Surgery: A Patient's Journey

This is the story of a woman (let’s name her Sunita ) who had suffered years of social, emotional as well as physical discomforts due to her large, disproportionate breasts. She always felt difficulty finding dresses that fit, distressed about being the center of unwanted attention, and suffered from physical ailments like neck, back, and shoulder pain. One day, she came to know that ‘Breast reduction surgery’ could effectively address her problems and improves the overall appearance. She did a Google search and collected a massive amount of information on breast reduction. She talked with her friends, and some of them advised that not all of the information found on the web is reliable. So, she started investigating deeper on the topic, discussed with various dermatologists and gathered at an inference of what actually breast reduction surgery is. Here we go with Sunita’s findings!

What is Breast Reduction Surgery?

Plastic surgeons  educated Sunitha that breast reduction surgery is a procedure carried out to remove the excess breast fat, glandular tissue and skin, in order to help her reduce the size and weight of her heavy breasts, thereby achieving a breast contour that is more in proportion with her body. They also informed her that the surgery could not only make her breasts smaller and firmer, but also improve the symmetry of the breasts.

Sunita then decided to undergo a breast reduction surgery and started hunting for a best plastic surgeon, who could help her gain a more proportioned bust line. Now, the next part of the story begins…

Choosing the Right Cosmetic Surgeon

This was the next key challenge faced by Sunitha. She searched for cosmetic surgeons’ websites and looked over their experiences in the relevant field. She contemplated various before and after photos on their websites and even talked with some of them.

And after days of deliberate thought, she chose a best plastic surgeon.

The Procedures Involved…

She consulted the doctor and had a detailed discussion on the steps of a breast reduction procedure.

  • Anesthesia
The Anesthetist inquired about her medical conditions and history, including whether she has had any medical problem affecting breasts. The doctor also delved deeper into the medical history of her family, any other medications she is on, allergies etc.

  •   Creating incisions
The surgeon told her that the next thing she will do is to make an incision around   Sunita’s nipple. The incision will generally be in the form of an inverted-T or anchor-shaped.

  • Repositioning nipple
The nipple will be then repositioned and underlying breast tissue will be reduced, lifted and properly shaped.

  • Closing the incisions
The final procedure will be to close the incisions using sutures, skin adhesives or surgical tape to support the newly shaped breasts.

Sunita  was then very clear on how the surgeon is going to reduce the size of her breasts. The next concern she had was that if the results of breast reduction surgery lasts or not.

The Results – Will it Last?

The doctor assured Sunita that she can expect to enjoy smaller, appealing breasts for a lifetime, as long as she could maintain a ideal  body weight .

Sunita was really happy to hear that and she decided to undergo a breast reduction surgery. So, the doctor advised her to:

  •  Complete several lab tests
  • Have a baseline mammogram
  • Stop taking aspirin or any anti-inflammatory drugs
  • Avoid smoking for a short period before and after the surgery
  • Make sure that she arranges someone to drive her home after surgery
Sunita  followed the advices of her doctor. And finally, she successfully underwent a breast reduction surgery!

Life After Breast Reduction?

Now, you might be curious to know what happened to our heroine after surgery, right? Well, Sunita, like most other patients, had a rapid relief from neck, back and shoulder pain after the surgery; however, she too had the typical issues like swelling, soreness, and tingling. As the doctor already advised her that the breasts often heal at a different rate, she was not worried to see some asymmetry during the first few months of recovery. Over time, the post-surgical swelling has resolved and incision lines have faded. After about 2-3 months, the final results were in place for Sunita. Years passed and now, Sunita is a self-confident lady with a renewed sense of freedom to wear the dresses she likes and participate in physical activities from which she was previously deprived of.

This is not just the story of Sunita; but the story of many women who have alleviated physical and emotional discomforts and acquired enhanced appearance through breast reduction surgeries with the help of the best cosmetic surgeons. If you are one among millions of ladies who are struggling with larger breasts, then it’s time that you get a breast reduction surgery from a  plastic surgeon.

Rhinoplasty – For All Who Expect More…!

“My nose is long and flabby at the end…” “Whenever I smile, my nose looks wide and ugly…” “I’ve got my nose distorted in an injury, and now I suffer from breathing problems everyday…” – these complaints are common. It’s a fact that half a million people are unhappy with the nose with which they were born, and are living with such worries… Rhinoplasty is the best solution for all such people – it is not just a procedure with complex series of steps, and it is not as bad as you may think! Here we are gonna review various aspects and possibilities of Rhinoplasty, which may help people who wish to, enhance the appearance of their nose.
Understanding Rhinoplasty…
Rhinoplasty is one of the most popular cosmetic procedures available today to reshape the nose. It involves making the nose larger or smaller, changing the symmetry or angle of the nose with respect to the upper lip, altering the nose tip, correcting the bumps, indentations, or other defects in the nose, to bring it into better proportion with the rest of your face. Rhinoplasty may also restore structural problems with the nose that cause chronic congestion and breathing problems.
Rhinoplasty are generally carried out by surgeons who are trained either in plastic surgery, otolaryngology (ear, nose, and throat specialty), or both.

The Procedures Involved…
During rhinoplasty, your surgeon will first access the bone and the cartilage support of the nose through smaller external incision made at the base of your nose, between your nostrils. Followed by this, the underlying bone and cartilage is readjusted to create a newly framed structure. For example, if you are dejected with the tip of the nose which is too large, your surgeon will sculpt the cartilage in this particular area, as a means to reduce its size. In case, if you feel that some portion of your nose is disproportionately small, cartilage or soft tissue grafts can be placed in order to create a more pleasing profile. Now, if the issue is that the bridge of the nose has a prominence, your surgeon will reduce this to yield better harmony. The angles of the nose in regard to the upper lip or the forehead can also be remolded by further sculpting the nasal support structure.
Next, the tissues are re-draped over the new frame and the incisions are closed. A small splint is then placed outside the nose to help maintain the new shape of the nose, as it heals.

Life after Rhinoplasty…
All rhinoplasty techniques are different – so, recovering time varies from patient to patient, somewhat depending on how much your nose is altered during surgery. However, here are few things, which you can expect during your recovery stage.
  • In the first week of rhinoplasty, it’s very common to have swelling and bruising around eyes and nose, sore throat, headaches, and nausea. Cold compresses and over-the-counter pain relievers will be recommended to help you abate these symptoms. Keeping your head elevated can also help to minimize swelling.
  • While it’s crucial that you follow your surgeon’s post operative instructions, ensure that you take your antibiotics religiously.
  • It’s still important that you should watch for any signs of infection, including worsening pain, redness, swelling, fever, red streaks or pus. If any such concerns occur, report immediately to your surgeon.
  • Avoid any activities that would increase the risk of injury (For example, heavy lifting, excessive exertion, sun exposure etc.).
  • It is indispensable to make sure that you don’t blow your nose and avoid any impact to the nose while the structures are healing.
  • If you are a person who wears glasses, you must be very careful not to exert excess pressure on the nose bridge.
  • Majority of the patients can return to work during the second week of recovery, as long as you do not work on any strenuous activities. At this time, gentle washing of the face can be resumed and a little makeup can be applied, as well (of course, that’s your choice!).
  • By week three, you can recognize the new changes and start appreciating the brand new shape of your nose!

Even though rhinoplasty surgery is a minor surgery, there are complications, risks and recovery time to consider, as with any surgery. However, with proper analysis, combined with the skills of a qualified surgeon, rhinoplasty will become a very rewarding procedure for all.

Facial rejuvenation - Restoring the triangle of Youth

Many of us are happy with the wisdom and composure we attain with age , But few are happy with the wrinkles, folds, jowls and deflation of the face. The pursuit for a youthful facial appearance may involve everything from healthy diet, exercise, facials, anti-ageing creams,  to lasers , skin tightening procedures, botox and injectable fillers. However with time the traingle of youth turns to the pyramid of age. Traingle of youth refers to the widest part of the face between the two cheek bones in young person with  the narrow point at the chin. With ageing, it reverses and the widest part becomes the jawline , with the narrow tip at the bridge of nose. This is because of the midface deflation, fat pad loss, bone resorption, skin wrinkles and jowls. Skin becomes thinner, collagen and elastin breaks down and hyaluronic acid is lost resulting in  crow’s feet, nasolabialfolds, tear trough and  marionette lines .Usually this starts around the age of 35 ,with further deflation and midface bone resorption at 45-50 years. Many of the non surgical procedures like botox, fillers , lasers help in defying the age, but are effective only upto a point.

Restoring the triangle of Youth

A modern face lift procedure is customized or individualized to patients needs because not every face  is the same. It not only lifts the deeper tissue and removes extra skin but also fills the midface volume loss with fat grafting. The windswept unnatural look  with lift-alone procedures has given way to the more pleasing and rejuvenated fuller facial profile with the current facelift surgery.

Mini facelift- Patients who exhibit a mild degree of jowling and sagging skin are often good candidates for a mini-facelift.This is a less invasive technique that allows to tighten deep facial tissues through shorter incisions, typically located along the natural creases surrounding the ear. It can be done under local anesthesia with sedation or under general anesthesia. Structural tissue around the cheeks are lifted and plicated to refine the jawline and restore the midface. Advantages include shorter scar, quicker recovery and lesser complications.

Standard face lift- This procedure will more fully address moderate to advance aging of the midface and neck .It involves incisions starting in the temple region extending  to ear running in front, around and then behind the ear and back into the scalp. Facial skin on the cheeks and neck is dissected free and underlying muscle and fascia of the face and neck is dissected and tightened. Excess skin is trimmed and the incisions are closed. It take around 4-6 hours for the surgery, recovery is longer but the outcomes can be rewarding.

Blepharoplasty or the Eye rejuvenation surgery-  Many a times the telltale signs of aging can first appear in  and around the eyes. This can be droopy upper eyelids and brows , puffy bags under the lower lids, or  sunken eyes and  tear trough. This can make you look old and tired and sometimes interfere with the vision too. Blepharoplasty is a surgery that removes excess skin ,plicates the muscle in upper lid to lift it up , and redistributes the fat in lower lid to attain a smooth lid-cheek junction. The incisions follow the natural creases of upper lid and just below the lashline in lower lid. A lateral canthopexy is also incorporated to tighten the outer part of eye. This can be combined with a brow lifting procedure for a sagging lateral brow.

Autologous fat grafting for facial rejuvenation- Fat transfer techniques using fat from the other parts of body and transferring it to elsewhere has become more widespread and established technique. It has been used for breast augmentation, buttocks augmentation( Brazilian Butt lift) , hand rejuvenation and also for facial aesthetics. Micro and nano fat harvested from abdomen or inner thighs are  injected in the planes of facial tissue with blunt cannulas. Clinical applications include  filling of the malar prominence, tear troughs, nasolabial folds and  fine lines along the lips. The  adipose derived stem cells in fat tissue are critical in tissue regeneration and repair. The effect lasts for 3-5 years unlike fillers which has to be injected every 6-9 months.

Adjuvant procedures for face

Thread lift- Bioabsorbablethreads with barbs or cones are inserted  underneath the skin in specific areas of face to lift the eyebrows, lift the sagging skin over cheek , reduce nasolabial folds, define jawline and reduce the neck folds. The procedure can be done with patient being fully conscious and they can approve the look of their thread lift while the surgeon works . This innovative technique assures a subtle yet a noticeable change .

Double chin Injectables- Submental fullness or  fat under the chin can be treated with injectable deoxycholic acid in 4-6 sessions spaced at 1 month interval. This treatment has been approved by FDA. Deoxycholic acid is a naturally occurring molecule in the body that aids in the breakdown of the fat cells resulting in a noticeable reduction in fullness of the chin.

Top 5 Myths about Plastic surgery

There are plenty of myths and misconceptions surrounding the field of Plastic surgery. But plastic surgery is very much like any other field of medicine with highly trained and specialised surgeons performing all kinds of reconstructive and cosmetic work. In this article I would try my best to clear some of the myths about Plastic surgery.

Myth #5-  Plastic surgery is all about glamour and beauty
Not necessarily. Though there are number of cosmetic procedures like breast augmentation, botox, rhinoplasty, liposuction to name a few, the field actually encompasses everything from correcting birth defects like cleft lip and palate to complex reconstruction following accidents to breast reconstruction after mastectomy. Plastic surgeons are trained to focus on restoring the function and form , and not just aesthetics.
Most often cosmetic surgery is resorted to if the correction is near impossible with nonsurgical methods.

Myth# 4- Plastic surgery is a scarless surgery and lasts for ever.
Very often patients ask me  if the scars are totally invisible after plastic surgery.  Yes best plastic surgeons are trained to deliver scars that are minimal /small and refined, but it does leave a scar after any type of plastic surgery. The scar formation is a long run process that can continue for an year. We can recommend scar treatments in the form of creams, nonsurgical methods and precautions to be taken after surgery.
Many of the cosmetic procedures lasts long if taken proper care after surgery. Plastic surgeons do decelerate the hands of clock, but the clock keeps ticking. Hence procedures like facelift, liposuction, tummy tuck can give you years with good skin care, surgical touch ups and nonsurgical interventions.

Myth #3 – All cosmetic surgeons are Plastic surgeons.
Unfortunately it is not so. Any person with a medical degree and license can practice cosmetic surgery in India. There are many clinics without a certified Plastic surgeon treating patients with liposuction, hair transplantation  and what not. So it has become a ‘buyer beware’ speciality.
Patients choosing a plastic surgeon should inquire about the training and certification , whether surgeries are done in accredited centers with safe anesthesia practice .

Myth #2 - Plastic surgery is only for women
It is not so now. A good number of  men are now opting for procedures to enhance their looks and  self esteem. While there a procedures like botox and fillers for a rejuvenated youthful face there are also more invasive surgeries like liposuction for gynecomastia ; rhinoplasty and facelift for a chiseled facial profile  or a abdominal etching for toned abdominal wall. Other common cosmetic surgeries that men opt to are chin implant for a firm jawline, liposuction for a double chin and body contouring after massive weight loss.

Myth #1- Plastic surgery is only for the rich and famous.
This is #1 myth about Plastic surgery. Most of the aesthetic patients are normal patients who want to look as good as they feel. This can restore confidence, overrall appearance and  improve their lives. Cosmetic surgeries opted for correction of deformities not amenable to nonsurgical methods is not going to burn a hole in your pockets. It is tremendously going to improve your self esteem -to name a few, correction of gynecomastia in a young man who is extremely embarrassed to wear any T-shirts, or a mommy makeover for a otherwise healthy lady with lax breasts and abdomen, or a breast implant in a young girl who is depressed about her body image.  Any of these above mentioned procedure is going to cost an affordable amount .

Yes it shouldn’t become an obsession to look good  and undergo repeated surgeries when it is called body dysmorphism.
